Please use the form below to send in your 2016 Q4 report. Please note a few changes with the new report format:

  • The new format calculates total amount due within the report, so there is no need to wait for a confirmation email.
  • The calculations include fundraising deductions for replacement of Ind*o (Prima) wraps. Remember, you can also use this deduction during 2017 Q1, so if you do not have a deduction to report now, you still have plenty of time.
  • After you complete the report, you will be redirected to PayPal to remit your payment. The amount due will show up in PayPal. Remember, if your total remittance calculation is less than $40.00, the minimum amount due is $40.00.
  • Please use your chapter’s PayPal account to remit payment. This will reduce delays in the payment recording to your chapter’s account.
  • You will immediately receive an email with a copy of the information you submitted in your report. Be sure to check your spam folder.
